CCAP Provider Registration
The Child Care Assistance Program requires that a child care provider be registered with the county in which the family receiving child care lives to be authorized to receive child care assistance payments. Lakes and Prairies CAP, Inc accepts and processes registrations for providers who serve families residing in counties that we service. If you want to be authorized in more than one county, you must complete registration forms for each separate county.

Licensed Provider Registration
Providers need to have a current license number before registering as a licensed provider.

Licensed Exempt Provider Registration
Providers considered to be exempt from being licensed by the state of Minnesota

Legal Nonlicensed Registration Packet
Provider must be at least 18 years of age. Provider is not a member of the MFIP assistance unit, or a member of the family applying for or receiving child care assistance. Provider is not living in the same home as the child whose family is applying for or receiving child care assistance. Provider provides child care only to related children, and/or provides child care to children from a single unrelated family at one time. Related refers to the provider being a sibling, a step-sibling, a niece, a nephew, a grandparent, an aunt, or an uncle.

This is a guide that explains what child care providers need to know to get payment from the Department of Human Services (DHS) for families who get benefits from the Child Care Assistance Program (CCAP).

Direct Deposit (DHS-3552)
Brochure explaining the benefits of directly depositing child care payments to bank accounts. The brochure includes a Direct Deposit Authorization form.

MECĀ² PRO Registration form (on-line billing)
Form to be completed in order to be enrolled in the on-line billing system. Providers will receive family billing forms on-line and be able to submit them electronically.

Form to be used if changes need to be made
to provider's current tax information.
